Deviant teenager( my stepson)?

how to handle a deviant 14 year old.For example, he will pull every clothing item out of his closet just to dump on the floor.He is over weight so we took him to a doctor to get a check up and so forth and he refused to get out of his dad's truck.We took him to a counselor and he just sat there not talking. We have tried positive reinforcement and negative reinforcement.We got him an ortment of yard jobs and he refused because the people did not have a riding lawn mower.For us he will push the lawn mower 10 feet and say he is done.He was suppose to help a elderly lady put her recyclables out , he took one hamburger helper box out to the curb and left.I think the best example of this was when he was crying his tooth hurt on a Sunday. We got the dentist in on a Sunday afternoon and he pulled the tooth.That week it was fine that following weekend on a Sunday , he is bawling his tooth hurts, again the dentist..nothing wrong with it, no infection, no sinus infection.The next day he claims there was nothing wrong with his mouth.I get chest pains from this child.What can we do?
